How does Benlysta work?

Benlysta is used mostly on patients with active autoantibody-positive SLE that are already receiving the standard therapy. It can also be prescribed for use on adults that have active lupus nephritis that are already receiving standard therapy. The medication is a sterile, preservative-free, lyophilized powder that is contained in a vial for dilution and reconstitution before intravenous infusion. It binds to the BLyS, or B lymphocyte stimulator, which means that the BLyS can no longer bind to the autoreactive B cells, leading to no stimulation of the said cells. These cells are what produces autoantibodies that attach to normal tissue and attack them.

Treatment Details

Benlysta should be diluted in 0.9% sodium chloride injection, USP with normal saline, 0.45% sodium chloride, USP half-normal saline, or lactated ringer’s injection, USP to a volume of 250ml for intravenous infusion. For patients weighing 40kg or less, 100 ml can be used so that the belimumab concentration inside the bag does not exceed 4mg/ml. From the infusion bag, withdraw and discard volume equal to the reconstituted solution of the medication into the infusion bag or bottle. The bag should then be inverted gently to mix the solution.

How long does Benlysta last?

The reconstituted solution should be stored in a place protected from direct sunlight and refrigerated at a temperature of 2°C to 8°C if not used immediately. The solution is administered via intravenous infusion over a period of 1 hour, and it should not be infused concomitantly in the same intravenous line with other medication.

Safety Information

Patients that have any known allergies or hypersensitivity to any of the components should not use Benlysta. Patients with HIV, a history of hepatitis B or C, a history of major organ transplant or hematopoietic stem cell transplant or renal transplant, or hypogammaglobinemia should avoid this medication.

Other Side Effects 

Any sign of allergic reactions should immediately be treated with emergency medical help. Some signs of allergic reactions within hours or days after the injection include:

  • Muscle pain
  • Tiredness
  • Slow heartbeats
  • Dizziness
  • Itching
  • Swelling face or throat
  • Nausea
  • Trouble breathing

Other common side effects include:

  • Diarrhea
  • Fever, stuffy nose, and cough
  • Pain in arms and legs
  • Headache
  • Sleep problems

Recovery Time 

It can sometimes take up to 8 weeks for a noticeable improvement in symptoms to appear, and four to eight months for the full effects to show. Once these do show, they usually last long-term.
